Job Description

Our aim is to have the best health and social care outcomes in the country delivered by one of the safest and most efficient systems; establishing an asset based approach to supporting patients and people in services that link physical and mental health, social care, general practice, broader primary care and the voluntary sector.

An exciting opportunity has arisen for an individual to work as part of a dynamic team providing administrative support to the Individualized Care Team.

The post-holder will undertake a variety of administrative functions, working closely with the wider team, which will include general office duties, as well more detailed work related to NHS Continuing Healthcare and Complex care for people.

The post involves frequent and on-going engagement with a range of individual’s including health and social care professionals, care providers and patients and their representatives and it is therefore essential that the successful candidate has effective communication skills.

The post-holder will be able to work autonomously, but also as a member of a team.


Required Knowledge, Skills, and Abilities
The post-holder will require the ability to respond to the challenges of the post and be adaptable, flexible and knowledgeable with regard to changes and demands within the service.
