Support the multi-disciplinary HUB team with the gathering and recording of information relating to patient discharge, recording agreed actions from twice daily pathway calls and undertaking enquires as and when is necessary.
Communicating relevant information to stakeholders and team members, taking messages and passing on accurately to the relevant departments/person, ensuring all urgent and/or confidential communications are received and distributed in a timely manor.
Undertaking administrative duties such as maintaining data bases, photocopying, arranging conference calls, arranging meetings and management of email.
Assist the Discharge Trackers and wider team in ensuring that up to date information regarding each patient's journey through the system and actions agreed is available and updated on a daily basis.
Amending data recording and collation as instructed by data analysists to ensure all discharges are tracked
To be fully conversant with all relevant IT based systems and software programmes, i.e. e-Mail, Internet, Intranet, Web based portals and Microsoft packages including, Word and Excel.
To organise and prioritise tasks and own workload to ensure the needs of the HUB team are met in a timely and effective manner.
To carry out other appropriate delegated duties as required.
Able to work on own initiative and unsupervised, within defined policies and procedures to set
To comply with Departmental, Divisional and Trust policies and procedures.
