Job Description

Working with the client, their families and litigation team to maxmise their rehabilitation in the family home, focusing on adaptions, managing a team of Support Workers and assessing and reviewing their needs assessments and risk assessments utilising our HR, Nurse Advisors and Training team.

We offer:

• £35,000 - £45,000 per annum pro rata depending upon experience;

Required Knowledge, Skills, and Abilities

A minimum 5 years’ post qualification; Experience of working with acquired brain, spinal or catastrophic injury; Experience of working within an MDT; Experience of working within the litigation process and Deputies; Excellent report writing skills (INA, Monthly summaries, risk assessments); Exceptional interpersonal and communication skills both verbal and written; To be self-motivated, organised, caring and have a passion for Case Management; Have experience of working with clients and their families with a long-term disability; Be a car owner and driver.
