Job Description

These new roles will help to support people and their families to maintain their independence, by offering a range of enabling help and support as required, tailored to the needs and wishes of the person. The roles will operate across a range of settings, including hospital wards and other care settings as well as people’s own homes. Typically, these roles will work with older adults and people who may be living with several long term health conditions or a disability.

Successful candidates will be joining a team of health and social care professionals and will have the opportunity to shadow and work alongside a range of disciplines as part of their career development. As a large organisation, we are also able to offer rotations across several service areas, allowing our colleagues the opportunity to pursue a range of interests and to develop into rounded practitioners.
