Job Description
  • Work within the Support Centre and, when required, undertake community visits for the purpose of assessments.
  • Collaborate with all disciplines as part of a multidisciplinary team (MDT) to ensure a high standard of therapeutic and rehabilitative care to attendees at the Support Centre.
  • Communicate effectively with a wide range of individuals, their careers, friends and family members
  • Report to and undertake regular supervision with the Support Centre Lead and to deputies in their absence.
  • Work in alignment with a Palliative Rehabilitative approach and the STAR assessment model
  • Coordinate the attendance of patients and/or their careers for engagement with activities and services provided by the Support Centre programme.
  • Coordinate the attendance of volunteers that support the day to day running of the Family Support Centre, including volunteer drivers that operate the Taxi Service
  • Provide a range of administrative support to the Support Centre Lead, including processing referrals and recording data on SystmOne.
  • Assist, when required, external facilitators with provision of service/activity within the Support Centre.
  • Attend regular staff meetings and MDT meetings as required and other external meetings as agreed with the Head of Support Services.
  • Contribute to the placement of students
  • Where required, assist with the administration and coordination of training programmes to internal and external colleagues

Required Knowledge, Skills, and Abilities
  • Experience of working in a healthcare setting, e.g., hospital, hospice care, home
  • Level 2 Healthcare (e.g. NVQ)
  • Good communication, numeracy and IT skills
  • Ability and desire to work independently and as part of a team
  • Be able to demonstrate commitment to Hospice Core Values and Care Standards
  • Ability to work effectively with highly emotive and demanding situations
  • Ability to use own initiative
  • A dynamic, enthusiastic and ‘can-do’ approach to problem-solving
