To have a strong working knowledge of the patient care process within the care systems.
To assist in the collection and recording of data relating to individual patient pathways. Communication of this information to relevant parties e.g. members, GPs, lead clinicians.
To assist with collection and analysis of data for specific audit projects when required, this may include the development of proforma’s, templates and other tools.
Knowledge of Trust Policies regarding Data Protection and security of Information and their application within the workplace.
To maintain accurate filing systems (electronic and manual) to ensure efficiency in producing desired information.
To order patient notes as requested.
To provide facilitation and administration cover when required administer for local MDT meetings. This may require cross site travel.
To document and circulate minutes of meetings, manage electronic diaries, book training and meeting rooms, organise related documentation, update attendance databases and support where necessary.
To be conversant with Infoflex database and Microsoft suite of applications.
Assist in Audits including data analysis and presentation.
To be flexible with working hours as the service requires- including some longer days
The post holder will be constantly dealing with data and listening to discussions about palliative/end of life care for patients, which at times can become emotionally demanding.
To attend non-clinical Mandatory Training in line with Trust policy
Have a full driving licence as some travel off site may be required
