Job Description

The post offers the opportunity to develop your skills and experience whilst making a difference to patient care. The post holder will need to demonstrate that they have good communication skills and that they are an excellent team player as well as being able to work alone. The role will be fast-paced and will require an extremely organized individual who can work well under pressure and who also has a flexible approach to the needs of the service.
This role will directly support our Intermediate Care Multi-Disciplinary Team providing vital administration support so the service can assist patients who require an urgent response to remain at home to avoid admission to hospital or expedite discharge.
The ability to travel between sites in a timely manner will be required.
