Job Description

You will work as part of committed and creative team, which offers innovative, holistic and person-aligned care and support to people with palliative and end life care needs, and to their families and care givers.

Duties include processing new referrals, gathering patient information, liaising with patients, careers and external health services, organizing patient transport, dealing with enquiries, assisting with service promotion, data entry and preparing of reports.

Applicants will have a background in providing high quality administrative support in a heath, social care or community service, be skilled at entering information in electronic medical records in an accurate and timely way and significant experience of engaging with people in distress with tact, diplomacy and sensitivity. The successful candidate will also possess excellent communication skills.

We offer 27 days holiday (pro rata) plus public holidays and excellent pension scheme.
