Job Description
You will be supporting the day-to-day running of the team that consist of a multi-professional skill mix. You will be highly organized and proactive with excellent computer and communication skills with a proven background working in an administrative capacity. You will be working alongside our clinical nurse specialists (CNSs), medical consultants, healthcare assistants (HCAs) and social worker to support the delivery of specialist palliative care in the community.
You role will include:
- Ensuring a professional and empathetic approach when dealing with patient/NOK calls
- Processing new patient referrals, following procedure to ensure information is gathered quickly and accurately, including Urgent ‘Red’ referrals that are communicated to the CNS in a timely way.
- Dealing with queries from District Nurses, hospital Palliative Care Teams, other healthcare professionals from acute settings, patients and their families.
- Maintaining the electronic patient record at all times.
- Facilitate effective communication between service users and team members.
